Accelerated Death Benefit Rider
*Included
This rider—also known as a Living Benefits Rider—is included in your policy at no charge. It gives you access to a portion of your policy’s death benefit, if you are diagnosed with a terminal illness resulting in 12 months or less to live.
Premium Waiver for Disability
*Available at additional cost
AXA will waive premium payments during a period of total disability lasting six consecutive months or more. The waived payments don’t have to be paid back. This rider is not available on all policies and the cost varies by age.
Child Rider
*Available at additional cost
For as little as $52.50 per year, add a child rider that covers your children under the age 18. Options range from $10,000 to $25,000 in coverage per child. Ask your Quotacy agent for details.

Manage your policy online
- Visit www.equitable.com.
- Click on the small Sign In link in the top right-hand corner.
- Click on Register Now.
- Click on Individual then choose Life or Investments.
- Enter your your email, Social Security Number, and date of birth to verify your identity.
- Click the blue Next button and follow prompts to set up your online access.

The Fine Print
Guidelines & Exclusions
Term Conversion
- May convert to permanent product before the final term’s policy anniversary or before age 70.
Accelerated Death Benefit Rider
- Also known as Living Benefits Rider.
- Accelerate up to 75% of your policy’s benefit—not to exceed $500,000.
- $250 processing fee when benefit is accelerated.
Child Rider
- Newborn child covered when 15 days old.
- Maximum coverage is $25,000 ($50,000 in New York).
- Rider not available if insured is given higher rating than Table D or a flat extra that equals or exceeds $15 per thousand.
- Parent (or child) can convert rider after child’s 25th birthday or before parent turns 65—whichever is earlier.
Premium Waiver for Disability Rider
- Not available on policies if insured is rated higher than Table D or a flat extra that equals or exceeds $10 per thousand.
- If total disability begins on or after age 60, premiums are waived until the policy anniversary closest to age 65 or termination of disability—whichever is earlier.
